In an effort to make the best use of your time and keep your waiting to a minimum, we have our welcome forms available to download that you can complete prior to coming to our office. If you are unable to fill out the forms before your visit, please arrive at least 15 minutes prior to your appointment time to allow time for you to do so in our office.

It is helpful to have the following items on your visit to our office: 

- Your current glasses or contact lens and sunglasses 
- A list of any medications you are taking 
- Your insurance card(s)
- A form of payment for your insurance co-pay
